Student Forms
Below you will find three blank forms for VA enrollment , notice of change, and transient permission. Please ensure you are completing and sending out the necessary form to the correct office. If you have questions, reach out to the Veteran Student Success Center.
Request for VA Enrollment Certification
Students must submit the VA Enrollment Request form in order to use Veteran Education Benefits. Students must submit this form each semester. The VA will not receive enrollment certifications without it. All students must read the Statement of Responsibilities document.

Notice of Change
Submit the required Notice of Change form to notify The Citadel’s Veteran Student Success Center of any changes to current enrollment. Submit this form as soon as possible for continued enrollment.
Cross Registering/Guest Students
The Transient Permission form is required for students who are attending another institution and wish to receive benefits at that institution. Have the form signed by the appropriate offices. Once all signatures are complete, a copy must be provided to The Citadel’s Veteran Student Success Center.
