Letter of Recommendation Form
This form should accompany a student's request to a faculty member for a letter of recommendation.
This information allows the faculty member to offer a more comprehensive assessment of the student, thus enhancing their overall application. Included in this form should be any additional information, such as copies of outstanding papers, publications, or presentations that the student feels is noteworthy.
Also included should be a copy of the student's audit. When asking faculty members to submit letters directly to a graduate institution or other agency, please make sure the address is included. Do not rely on the recommendation form since not all of them include the address on their forms.
Finally, please allow at least three weeks for the faculty member to complete your letter.
