(Please use an email address you will check frequently.)<\/p>\n\n\n\n<p>Directions:<br>1. Student &#8211; log in to DocuSign and complete all forms (DocuSign creates an electronic signature that will be used on the forms).<br>2. Parent\/guardian &#8211; receive an auto-generated email from DocuSign. <br>3. Parent\/guardian &#8211; click on the link provided in the email from DocuSign to review, sign and initial forms.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-medical-clearance\">Medical Clearance<\/h2>\n\n\n\n<hr class=\"wp-block-separator has-css-opacity\" \/>\n\n\n\n<p>All students who have been academically accepted to The Citadel will receive instructions from <em>MUSC Health at The Citadel <\/em>on how to complete medical clearance. <strong>This<\/strong> <strong>must be completed prior to attending CSI<\/strong>.
