If you will be a minor (under 18) on the date you report
for CSI you must either:
(a) have your parents/legal guardian complete and sign the following legal waivers and documents before you arrive for CSI and send them back to The CSI office or bring them with you when you check-in for CSI
or
(b) your parents/legal guardians must be present with you at check-in to sign the required legal waivers and documents.
