Letter of Recommendation
Form
This form should accompany a cadet’s request to a faculty
member for a letter of recommendation. This information allows the faculty
member to offer a more comprehensive assessment of the cadet, thus enhancing
their overall application. Included in this form should be any additional
information, such as copies of outstanding papers, publications, or
presentations that the cadet feels is noteworthy. Also included should be a
copy of the cadet’s audit. When asking faculty to submit letters directly to
the university, please make sure the address of the graduate institution or
agency is included. Do not rely on the recommendation form since not all of
them include the address on their forms. Finally, please allow at least three
weeks for the faculty to complete your letter.
