Documentation:
For students with disabilities

Under the Americans with Disabilities Act (ADA) and section 504 of the Rehabilitation Act of 1973, individuals with disabilities are guaranteed certain protections and rights to equal access to programs and services.  All accommodations are coordinated through the office of Academic Support.  The Director of OASIS reviews requests for accommodations on a case by case basis.  In order for accommodations to be granted, the student must be able to demonstrate the disability substantially limits their potential to learn.  The following outlines the procedure for requesting accommodations and the materials required to support such a request.

1.  OASIS must be provided with the student's psycho-educational evaluation.  If an evaluation does not clearly meet the following guidelines,  clarification, and, if necessary, more information may be needed.  The evaluation must:

  • Be current (administered within the last 3 years) and,
  • Be conducted by a qualified professional (school psychologist, clinical psychologist, nueropsychologist) and,
  • Provide actual test scores, including standard scores, percentiles, and/or grade equivalents form the standardized instruments administered and,
  • Include an interpretive summary of results and recommended accommodations.
  • 2.  The student must schedule an appointment with the Director to discuss his/her academic needs. At this time, accommodations will either be approved or other courses of action recommended.

    3.  In instances where accommodations are granted, the students are instructed on their rights and responsibilities concerning their accommodations and are required to take the necessary documentation to their respective professor.
