Before you may practice or compete, a Player Packet must be on file in the Club Sport office. You may complete, print, and deliver forms, or come by the Club Sport Office to complete them. If you need help completing a form, we will be happy to provide this. You may use a telephone and/or fax machine in our office to contact a parent or guardian.

You are responsible for ensuring that your Player Packet is complete and accepted, and that you are eligible to participate.
Your team captain will receive periodic updates about player eligibility, or you may contact the Club Sport Office. |

Each club sport is required to collect dues from its members. The amount is set at the start of each year by team leadership. See your team's website for details.
Dues are not QM-able. You must make payment in cash or with a check (made out to your team). Dues are collected the Club Sport office: do not give your dues to any member of your team. Bring them to Deas Hall. |

Before you may practice or compete, you must pay your dues. If you are not able to pay all dues owed at one time, contact the Club Sport office. We will set-up a payment plan that is not only agreeable to you, but also meets your team's expectations.
