Account Request Form


Account Requests for New Employees Starting at The Citadel:

Please review the guidelines in this step-by-step checklist before requesting accounts that access Citadel email or on-line systems.

If you are requesting account access for a new or existing employee, please complete all sections of the form below. If the employee is paid by The Citadel's payroll, appropriate Human Resources paperwork must be completed before the account(s) can be created.

If the employee is hired to work at The Citadel but will not be paid by The Citadel's payroll, (for example, active duty military faculty or Aramark employees), please submit the form for the employee and then direct the employee to contact Patti Becker, ITS Departmental Secretary for assistance. Patti initiates the account creation process for all non-payroll personnel.

Note: The Citadel hosts accounts for currently enrolled students and for current employees (faculty or staff); retired employees (faculty or staff) do not have access to Lesesne Gateway or other accounts.

Account Request for Existing Employee:

If the employee who needs access to another system is not already using Citadel email or logging into Lesesne Gateway, complete the request as if this is a newly hired employee.

If the employee already uses Citadel email and has access to other Citadel systems but must now use an additional system, please submit this form: account request form for existing employees

B. Affiliation

Campus-Wide ID Is this employee currently on the Citadel's payroll? Is this a contracted or temporary employee? If yes, provide end date of current assignment and/or military orders. Primary Employee Affiliation


Is this a current Citadel student?

C. Lesesne Gateway and Citadel Email

Lesesne Gateway portal login will be provided by default to enable employee access to check payroll information and update emergency contact information for the BulldogAlert system.

Citadel email accounts are also created by default and must be used by employees to disseminate Citadel information. If this employee does not want to use email at all, please indicate that here and a Citadel account will not be created.

D. Access to Authorized Systems

Department Head or Director who authorized this request:


Has the department head authorized this person to access the Class Absence System (CAS)? Has the department head authorized this person to manage finances, requisition, or approve items purchased with Citadel funds?
Has the department head authorized for this person to have access to Special Orders System (SOS)?

 

 

E. Computer and Resources:

Will this person use a Citadel-owned,
grant-funded or DOD computer?
Indicate the current location of the computer Please provide building name and room number of
shared office or shop if multiple people share the computer. Select the "Kiosk" option if there is no computer available in office/shop/suite.
Does the user need any special
software applications or access to any departmental network printers?
When will the employee arrive on campus?
(Preferred date for computer to be configured and ready to use)
Please provide any additional information about the computer, if appropriate.

 

F. Contact Information and Coordination:

ITS will create the appropriate campus network login/portal/email account using the information provided above within 1-3 business days of receipt of this request.

When the account is ready to use, we will notify the account owner via personal email and include login instructions.
