SUBJECT: The Citadel’s Policies Pertaining to Your Rights with Respect
to the Family Educational Rights and Privacy Act (FERPA)
The contents of this section of the White Book are provided for your use pertaining to your rights under FERPA while a cadet at The Citadel.
1. The first enclosure to this Appendix is the 2007-2008 Citadel Policy on the Confidentiality of Student Records. This policy outlines The Citadel’s FERPA policy, and can be found in The Citadel catalog and as a link from The Citadel’s web site.
2. The second enclosure is the AY 2007-2008 Annual FERPA Notification to Students. This notice outlines the rights that students at The Citadel have under FERPA.
3. The third enclosure is General Order No. 6, dated 26 July 2007, which appoints Mr. Mark C. Brandeburg as The Citadel’s Privacy Officer for AY 2007-2008.
4. The fourth enclosure is General Order No. 8, dated 11 June 2003, which is The Citadel’s Policy on the Security and Confidentiality of Student Records. This order specifies the internal security measures that are required for educational records maintained by The Citadel.
5. The fifth enclosure is Memorandum 4, dated 18 August 2006, which is The Citadel’s Information Security Plan for Student Records. This memorandum identifies the record custodians for all educational records at The Citadel, their responsibilities and Standard Operating Procedures for the handling of records.
6. The sixth enclosure is a copy of the “Release Authorization” that a cadet must sign in order to provide written consent for release of medical and health information contained in educational records maintained by The Citadel to third parties.
7. The seventh enclosure is a copy of the “Release Authorization” that a cadet must sign in order to provide written consent for release of disciplinary information contained in educational records maintained by the Citadel to third parties

8. The eighth enclosure is a copy of the “Release Authorization” that a cadet must sign in order to provide written consent for release of general educational records and information maintained by The Citadel to third parties
9. The ninth enclosure is a copy of the “Infirmary Information Release Form” that a cadet must sign in order to provide written consent to the release of information concerning a cadet’s location, medical disposition, and duty restrictions to individuals on a need-to-know basis.
10. The tenth enclosure is The Cadet Leader’s Statement of Understanding of Responsibility under FERPA. This form must be completed by a member of the chain of command after he or she has had the appropriate FERPA briefing by the Privacy Officer or his representative and before the cadet leader can be granted access to the educational records of other students.
11. The eleventh enclosure is The Cadet Academic Officer’s Statement of Understanding of Responsibility under FERPA. This form must be completed by an academic officer after he or she has had the appropriate FERPA briefing by the Privacy Officer or his representative and before the academic officer can be granted access to the educational records of other students.
12. The twelfth enclosure is the Authorization for the Release of Information by the Counseling Center.
