Graduating Seniors. This form will assist us in tracking information vital to our departmental assessment efforts. We are required to conduct assessment exercises on an annual basis. All chemistry majors who are planning to "walk across the stage" during the upcoming Commencement ceremony should fill out this form prior to Finals Week. Please note that your e-mail address will accompany the form.
Incoming Students. This form will allow us to collect information that will assist us in advising new students. The information will be forwarded to the faculty member that is assigned as the student's advisor. Please note that your e-mail address will accompany the form.
Information Update. During our advising process, a departmental committee meets with all rising juniors and "at risk" students identified by classroom professors or advisors. Because these meetings are designed to assist students in meeting their career goals, we need to have as much information as possible. All students who are slated to appear before this committee should fill out this form in advance. Additionally, anyone wishing to update the information in their files are welcome to submit a form at anytime. Please note that your e-mail address will accompany the form.
