This page will contain forms and other aids to the faculty to meet the departmental guidelines for assessing our programs. You may fill these forms out online and print them when you are finished.
Seminar Evaluation Forms. Fill
out this form and immdiately print it. Initial the top page that has the scores
on it and turn in both pages. DO NOT initial the page that has the comments
on it as it will given to the student. Once you have printed the form, press
the submit button to send the data to me. If there is a problem, please let
me know.
Student Portfolio Forms. This is the form that
should accompany all major works in the departmental student portfolios. Print
the form on YELLOW paper and attach to the
assignment.
Program Assessment Form. This is the form that is used to summarize the materials in the student's departmental portfolio. Normally, only the Department Head or Chair of the Assessment Committee will need to use this form. Print the form on GREEN paper and place in the student portfolio.
